Illinois Fire Department Tax Filing
with the Illinois Municipal League


All filings are due by: August 31, 2011

Foreign Fire Insurance Companies must file with the Illinois Municipal League an electronic schedule listing the names of municipalities and fire protection districts receiving the 2% Fire Department Tax and the amount of tax due to each.

This year we have converted to electronic filing only.


Process for electronic filing:
  1. Review the Instructions completely, as changes have been made from 2010.

  2. Review the Bulletin, as some code numbers have changed from 2010 and there are new city/village/FPD’s filing for the first time in 2011. The complete list of codes is available in the 2011 Illinois Fire Department Tax Code Numbers Booklet.

  3. Complete the Excel spreadsheet schedule provided by the IML. Information may also be submitted in any other electronic list (delimited text or Excel spreadsheet preferred) so long as it shows:

    1. Code Number
    2. Name of municipality or fire protection district
    3. Fire Premium
    4. Tax Due

    DO NOT put multiple insurance companies in a single spreadsheet. Please submit a separate spreadsheet document for each insurance company.

    If you have questions pertaining to alternate forms of electronic filing please email or call the League office at 217-525-1220.

  4. When complete, email your attached schedule with the Insurance Company Code Number in the message body.

  5. Complete the Sworn Affidavit Signature Page and mail it along with your check, payable to the "Illinois Municipal League Fire Tax Fund", for the total tax due to the address below. The total amounts listed on the Sworn Affidavit Signature Page should match your totals on the spreadsheet. Your company must submit one check for the total amount of tax due to municipalities and fire protection districts.

    Illinois Municipal League Fire Tax Fund

  6. After the IML has completely processed the Schedule, Sworn Affidavit Signature Page and check, an acknowledgment letter will be mailed to you as confirmation. All amounts of tax submitted to the IML by insurance companies will be submitted to the Illinois Department of Financial and Professional Regulation – Division of Insurance.


Schedules cannot be accepted where credits have been deducted. The Illinois Municipal League forwards all payments to the individual jurisdictions and any requests for refunds should be made directly to the municipality or fire protection district.

The fire portion of homeowners, commercial multi-peril, etc. policies should be included in the schedule.

We will send one check to each municipality or fire protection district, together with a list of all companies making tax payments.


Receipts

A listing of all insurance companies' total fire premiums and tax for each municipality and fire protection district is retained in this office. The listing is available for inspection by anyone concerned during regular business hours.
